What is MIGRAINE!!!

Migraine is a common, harmless, peculiar disorder of the nervous system, which results from changes in the blood circulation in the brain. These changes in circulation produce all the symptoms (so the spots before the eyes are actually happening in the brain!) Migraine usually runs in families and seems to be more common in women.

Migraine episodes are usually triggered by some kind of stress: usually from fatigue, or nervous tension, or food allergy. They most often occur in people who are intelligent, hardworking, and compulsive. Often migraine patients are a little too hard-driving, perfectionistic, and rigid; they may be pushing themselves too hard. (Sometimes the migraine episode is Nature's way of telling us to take life a little easier, to be a little less strict with ourselves.) Most of the great names In history, the world’s great "achievers", were migraine sufferers.

HOW DOES IT HAPPEN?
While we do not have all the answers, it appears that when a predisposed person is sufficiently stressed, certain chemicals are released by the body. Alter a variable period of lime (which explains why the episode does not always promptly follow stress) these chemicals produce charges in the muscles of the blood vessels that supply the circulation to the back of the brain (the area where we actually "see").
At first the muscles tighten in spasm and the blood flow slows: this causes a disturbed electrical current to move across the back, (or seeing pad) of the brain. It takes about twenty minutes for this disturbance to cross the visual area of the brain, and this corresponds to the time most patients see the "twinkling spots" before their eyes. Usually, the electrical disturbance stops at the end of the visual area, but if it goes further, numbness and tingling may arise in the face, arms, or legs. Nausea or vomiting may occur.
Next, the blood vessel muscles relax abruptly. This allows the vessels to stretch and dilate, producing the pulsating ("pounding") headache, usually on only one side of the head. If the muscles relax more slowly, the headache does not occur.

IS THERE ANY TREATMENT?
The first and best treatment is to understand your symptoms and to relax and be interested in your condition. For example, some patients have migraine episodes whenever they miss breakfast, others when they get too little sleep, some when they eat certain foods (sugar, MSG, and wheat products). Some persons have episodes only on Mondays, or when they are angry, or when viewing certain visual patterns (op art, etc.) Women have more symptoms when pregnant or while taking birth control pills. Most people who only have migraine episodes once every year or so, will have no interest in keeping track of the occurrences. But if you have recurrences, say, every month, you may find it helpful to keep a diary of the time of the day, duration, and associated activities, in order to analyze the frequency and causes. I will be pleased to supply diary sheets and review your diary with you. Most people find that migraine episodes are troublesome for several years, and then become less frequent with the passage of time: so do not be discouraged.

DO I NEED MEDICINE?
Medication is available to try to ease the headache episodes that follow the scotoma. Often this works, but the medicine must be taken quickly at the first hint of an attack or it will do little good. Other medicine is used as a preventative for those unfortunates who have very frequent headache episodes. But 99% of migraine patients need no medicine, only the reassurance that their bodies are simply signaling that they are a little out of tune. So relax and listen to your body, it may be telling you something.

WHAT ELSE SHOULD I DO?
While everything seems perfectly normal at this time, you should advise me or your family physician if you notice any change in your own usual pattern or frequency of symptoms. Occasionally, we find it necessary to “escalate” our investigation through the use of special neurological consultation, X-rays, etc. If you are not sure whether a change in your symptoms is sufficient to suggest further evaluation, please call me to discuss it. Finally, remember: your eyes are fine; you are not losing your vision. Most famous people have scotomas like yours, usually because they set unrealistic goals for themselves and others. The worst thing you can do is worry. Relax, and "beyond a wholesome discipline, be gentle with yourself."
